We currently work with PPO plans. If you would like us to verify your insurance benefits, please email photos of the front and back of your insurance card, along with your full name and date of birth, at least one business day before your appointment.

Please complete the Health History & Consent Form prior to your first appointment.

We recommend eating a light meal before your session and wearing loose, comfortable clothing.
